All minor hockey bench staff members are required to have a valid Vulnerable Sector Check. If applicants do not have a current VSC they may still apply however must immediately submit to the OPP a request for the VSC be completed.

To be considered as a Volunteer candidate all applicants must understand and agree to the following conditions:
1. Accept full responsibility for the team conduct, finances, team officials, etc.
2. Operate within the SDMHA Coach Code of Conduct.
3. Complete a Vulnerable Sector Check that returns with a no record result.
4. Agree to verification on any information provided on this application form.
5. Agree that all coaching qualifications are complete prior to Hockey Eastern Ontario deadlines.
